I've got the service record book for my 2003 Foxeye.
It's a little white book about 4"X3". I didn't think it was specific to the Fazer though. I thought they used the same booklet for all models of that era.
I can post pics of the front and back when i get home on Saturday.

No worries. I'll do the front and reverse + a couple of the service pages.
Like Disorderlypunk says though, there's probably not many left with the service book now. Especially if they've changed hands a few times.
