Somethings not right

So atm I feel the gear change (mainly first to second) is very sloppy, plus on and off the throttle on these two gears turns the bike into a seesaw, I've posted about this recently thinking it might be chain slack, but that is all adjusted so unsure of where to go from here? My thoughts are:

A. Issue with something in the gearbox?

B. I have an extra tooth on the front sprocket so around town (my main commute) the first 2 gears are too long and in lower revs have zero drive, hence no chain tension.

C.  A dieing clutch? Feels unable to really hold the engine at lower RPM, but again that could be the hearing more than anything.
