Firstly, look at the wear on it, if it's more worn to one side it could simply be that that's what's causing the problem.
Secondly, I'd check the forks are straight and the headset isn't loose.
Finally have a look at the where the forksĀ mount at the top of the yokes/ headset and make sure they're at the same level.
