This could be a few things: -

1. Dry chain
2. Misaligned wheels causing the chain to run out of line chain.
3. Tight spot caused by either of both of the above.
4. Loose front sprocket . 

If (1) is allowed to go on long enough it will bugger the chain/sprockets and you get (3) if (2) is allowed to go on long enough you get (3) if bad enough (3) will spell the end of the chain/sprockets.  (4) is not uncommon on early Fazers but very unusual on the injected normally caused by poor fitting.
