If the bike runs o.k. with no blue smoke it sound to me like you'r being paranoid.
Two pistons are going up as two are coming down but there will be some air movement as the pistons thrash about.

Its never even crossed my mind to run the engine with the cap off.......why do it out of interest?
Easiest way to go fast........don't buy a blue bike
